Here are the CPT codes for implants:
21248 - Reconstruction of mandible or maxilla, endosteal implant (eg, blade, cylinder); partial(3or less)
21249 - Reconstruction of mandible or maxilla, endosteal implant (eg, blade, cylinder); complete (4 or more)

For the crown you can bill with the dental code which many insures will accept or you can use CPT 41899 dentoalveolar procedure, ulisted and be sure to include a full description in your narrative of the service you provided.

Remember when billing medically you will also need to provide a diagnosis code. A great code would be K00.0 Anodontia

Hi, I have an 18 y/o patient who has a congenitally missing tooth for area #20. We are placing a dental implant and then the restorative crown. He has medical coverage with BCBS. How would I go about coding for both procedures? Thanks for any help you can offer.
