Great question. These follow up visits post 90 days of the delivery of a custom-made intra-oral appliance for OSA are typically billed to a patients medical insurance using the established patient Evaluation & Management (E&M) codes (aka office visit codes) - 99211=99215:
99212 – 10-19 mins
99213 – 20-29 mins
99214 – 30-39 mins
99215 – 40-54 mins

There is separate a code available for repair if that is something that occurs during a follow up:

K0739 - Repair or nonroutine service for durable medical equipment other than oxygen equipment requiring the skill of a technician, labor component, per 15 minutes

Is there a specific medical code for D9948— Adjustment of custom sleep apnea appliance? We would like to bill medical for an adjustment to the OSA appliance, this is outside of her initial 90 day period.
