There is not a specific CPT code available we are aware of for guided bone regeneration in the jaw at this point in time, so the closest CPT coding options available we are able to locate (since bone grafting is used as part of this procedure):

21210 - Graft, bone; nasal, maxillary or malar areas (includes obtaining graft)
21215 - Graft, bone; mandible (includes obtaining graft)
**use modifier -52 for reduced services when bone is not obtained from patient

Or, unlisted procedure codes may be considered, and a narrative report explaining what procedure the code is being used to represent can be included:

21299 - Unlisted craniofacial and maxillofacial procedure
99199 - Unlisted special service, procedure or report

Can you please assist me with the following CPT codes required by my medical insurance provider. I had major surgery due to a medical emergency as my jaw became perforated from an infection

I required Guided BONE Regeneration (which is different from Guided Tissue Regeneration) for 3 areas.
I would greatly apreciate your help with this.
