Hi bethjones!

J0588 is a valid codes for botox (as you found, J0858 is not).

J0588 stands for: Injection, incobotulinumtoxinA, 1 unit [Xeomin]

The other 2 possible codes for botox type A are:
J0585 - Botulinum toxin type A, per unit [Botox]
J0586 - Injection, Abobotulinumtoxina, 5 units [Dysport]

Now - as for entering the NCD number - are you entering it into field 19, or onto the red shaded portion of the line item that the J code appears on? I have seen different insurers request that in either area, so it may be a good idea to include it on both areas just in case :)

I am trying to bill botox injections for TMD I am using the code J0588 with the NDC on the vial but my claims are all rejected from the insurance stating NDC number. Procedure code of services rendered.
I had previously tried billing J0858 but it denied as well. I need help ASAP with this issue.
