Here are some CPT cross coding options for you:

40490 - Biopsy lip
40808 - Biopsy, vestibule of mouth
41108 - Biopsy of floor of mouth
41110 - Biopsy of tongue, anterior two-thirds
41105 - Biopsy of tongue, posterior one-third


D9222 – deep sedation/general anesthesia – first 15 minutes
and
D9223 – deep sedation/general anesthesia – each additional 15 minutes
can be crosscoded to:
00170 - Anesthesia for intraoral procedures, including biopsy; not otherwise specified

The diagnosis code(s) will depend on the patient's condition/symptoms - so in other words, why is the patient receiving the services?

For example, you may consider:
