I'm trying to file a medical claim for the below.

What diagnoses codes would be used for : Facial trauma, Teeth 7, 8, 9, 10 broke from fall into tile[/b]?

What medical procedure codes for:

D0230- intraoral - periapical each additional radiographic image
D0220 -intraoral - periapical first radiographic image
D3310 - endodontic therapy, anterior tooth (excluding final restoration)
D2335 - Incisal angle to be defined as one of the angles formed by the junction of the incisal and the mesial or distal surface of an anterior tooth.
D2332- resin-based composite - three surfaces, anterior
D2335 - Incisal angle to be defined as one of the angles formed by the junction of the incisal and the mesial or distal surface of an anterior tooth.
D2740 - crown - porcelain/ceramic
