Based on what you described, here are some coding options for you:

Diagnosis:
D10.0 - Benign neoplasm of lip

Procedure:
40490 - Biopsy lip
11440 - excision benign lesion 0.0 to 0.5 cm
11441 - excision benign lesion 0.6 to 1.0 cm

We removed/biopsy a fibroma from the lower lip what are the correct medical codes (the CPT & the ICD)
